Membership Terms & Policies
(Apply to all memberships unless otherwise stated)
Commitment
Olympus and Wellness memberships require a 3-month minimum commitment.
Baker membership requires a 12-month minimum commitment.
After the initial commitment period, memberships continue on a month-to-month basis unless cancelled.
Cancellation
Membership cancellation requires 30 days written notice via email prior to the next billing cycle.
A cancellation form will be sent and must be completed to finalize cancellation.
If notice is not received 30 days prior to the next billing cycle, the following month will be billed.
Early Termination
Baker Membership cancelled before the 12-month commitment is completed will incur a $500 early termination fee.
Billing
The first payment is due at enrollment.
Membership payments will be automatically charged on the 1st of each month to the card on file.
All membership payments are non-refundable.
Membership Usage
Memberships are non-transferable and may only be used by the individual enrolled.
Monthly benefits may not be shared or gifted.
Appointment Policy
A 24-hour cancellation policy applies to all appointments.
Missed appointments or no-shows may result in the monthly benefit being considered redeemed.
Auto-Payment Authorization
By enrolling in a membership, you authorize Rejuvenate Wellness and Aesthetics to automatically charge your designated credit card, debit card, or bank account for your monthly membership fee on a recurring basis on the first day of each month.
Late or Failed Payments
If a payment cannot be processed, you authorize Rejuvenate Wellness and Aesthetics to:
Retry the charge up to three times within 10 days
Contact you by phone or email to resolve the issue
Apply a $15 late fee for payments more than 5 days past due
Failure to resolve payment within 15 days may result in suspension or cancellation of membership benefits.
No Chargebacks
Members agree not to initiate chargebacks or payment disputes without first contacting Rejuvenate Wellness and Aesthetics to resolve the issue directly.
Unauthorized chargebacks may result in:
Immediate membership termination
Referral to collections if applicable.
Billing Information Updates
Members agree to notify Rejuvenate Wellness and Aesthetics of any billing information changes at least 5 days before the scheduled payment date.
Acknowledgment
Enrollment in a membership indicates that you have read, understand, and agree to the membership terms, policies, and financial obligations outlined above.

the fine print
- When signing up, we require a 3-month minimum to join
- After the 3-month period, If you need to cancel your membership, we ask is that you give us a thirty day written notice via email- we will then send you a cancellation form to fill out
- Your first monthly membership fee will be due at the time you sign up. Recurring monthly payments will be automatically charged to your card on the first of the month
- Our wellness club covers up to $169 of an iv drip for the month. If you select a drip that is over $169, an upcharge will be issued at your scheduled appointment to cover the difference.
- Additional drips during the month will be $139, as well as an upcharge if applicable. The fee for the additional drip will be issued at your scheduled appointment.
- Your membership benefits may be used only by you. You may not share or ‘gift’ your monthly iv drip or additional benefits
- If you do not attend your scheduled appointment, your membership is considered redeemed for the month. No exceptions
- If you are not able to make it in, your unused iv drip will roll over to the next month. Maximum thirty days
